SERVICING YOUR ACCOUNT

policy or quarterly statement handy to select the
correct forms.)

Use this form to update your existing plan, such as address change,
increase/decrease deductions, name change, beneficiary change, etc.
Use this form for a direct rollover/transfer another TSA, TRS
monies, sick-leave, 401(k), etc.
Use this form to rollover/transfer an IRA to your Transamerica IRA
account. If you are establishing a new Transamerica IRA account, use
this IRA transfer form in addition to the TransMark 1200 IRA
Application.
Use this form to transfer non-qualified funds to your Transamerica
Non-Qualified account. Non-qualified funds are all other monies that
are not coming from an IRA or a TSA qualified plan, such as CD's, bonds,
money market account and savings account funds, etc. If you are
establishing a new Transamerica Non-Qualified account, use this form in
addition to the TransMark 1200 Non-Qualified Application.
Use this form to establish a new Transamerica single premium
account. You may set-up the account as either a TSA, IRA or
Non-Qualified account, with the appropriate Rollover/Transfer Form
attached. Minimum single premium to open the account is $5000.
Use this form to request a loan against your existing TSA account.
The minimum loan check is $1000 up to 80% of your accumulated cash value.
You must have approximately $1800 in the accumulated cash value to be
eligible for the minimum loan. Loans are to be paid back via
check/money order. Please allow 7-10 business days to process the
application upon receipt. Call Customer Service for further detail
or account value at 1-888-565-7171.
